Es necessita un/a treballador/a social per a residència a Barcelona

- Establir el primer contacte i fer l’acollida de la persona usuària i de la seva família. - Dur a terme la valoració social i elaborar, conjuntament amb l’equip, el Pla Individual d’Atenció Interdisciplinària (PIAI), així com fer-ne el seguiment. - Intervenir en situacions de conflicte, pèrdua o final de vida. - Donar suport en gestions socials. - Acompanyar en l’accés a recursos externs i informar sobre qüestions com les voluntats anticipades o les tuteles. - Participar activament en les reunions de l’equip interdisciplinari, aportant la perspectiva social per millorar l’atenció integral. - Col·laborar en l’elaboració i revisió de protocols i plans de millora. - Coordinar-se en la planificació de les activitats del centre. - Mantenir una relació fluida amb els serveis socials, entitats i voluntariat de l’entorn, afavorint la interacció del centre amb la comunitat.

Experiència 1 anys. Es requereix experiència mínima d’1 any. Valorable experiència amb persones dependents.

Diplomatura/Grau en Treball Social homologat a l’estat espanyol

Competències / coneixements: - Desitjable formació en geriatria. - Informàtica: coneixements a nivell d’usuari. Experiència en l’ús de la plataforma AEGERUS.
