Role type: Max term contract, part time (16hrs per week until July 2027) with the potential for extension

Role grade: Pharmacist Grade 1 (year 1-5) to Pharmacist Grade 2 (Year 1-3)

Salary: $83,734.08 - $123,940.80 pro-rata + Super + Salary Packaging

About the role

Hospital in the Home (HITH) provides hospital-level care outside the traditional hospital setting, delivering short-term, high-intensity treatment through a multidisciplinary team to support patient recovery, improve outcomes, and reduce avoidable hospital admissions

This is a part-time position of 16 hours per week, worked across Thursdays and Fridays, with the opportunity to work additional hours to assist with sick leave and operational coverage when required

Provide comprehensive pharmacy assessment, clinical interventions and medication management for Hospital in the Home (HITH) patients, including the facilitation of ongoing intravenous medication management where required

Work collaboratively within a multidisciplinary team and with external healthcare providers, including GPs, community pharmacists, community services and hospital providers, to ensure coordinated, high-quality patient care

Provide education and expert advice to patients, staff and the community on medication management, promoting the Quality Use of Medicines, appropriate use of medication compliance aids, and client independence

Contribute to service improvement through quality improvement activities, clinical education, effective caseload management, and participation in team meetings and policy development

About you

Current unrestricted NSW Drivers License

Current registration as a Pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA), with no conditions on scope of practice, and a minimum of three years' post-registration hospital pharmacy experience

Demonstrated clinical knowledge of medicines, including antimicrobial stewardship and Quality Use of Medicines (QUM) principles

Demonstrated experience in a similar role, preferably within a community-based or community health setting

Demonstrated high-level clinical assessment, critical thinking, and problem-solving skills, with the ability to deliver safe, high-quality, person-centred care
