Clinical Nurse Specialist - Respiratory
About the role
We are seeking a motivated Band 6 Respiratory Specialist Nurse to support delivery of high quality, patient centered care across acute and community pathways. The role will provide expert assessment and management for patients with complex respiratory conditions, contributing to admission avoidance and reduced length of stay. You will work closely with the multidisciplinary team to improve patient flow, enhance clinical outcomes, and support ongoing service development.
Demonstrate empathetic interpersonal and communication skills in dealing
with patients and carers through the patient pathway.
Demonstrate advanced practice in holistic assessment, planning,
implementation and evaluation of care within the specialism. This will necessitate high levels of autonomy and advocacy and include receiving direct referrals and ensuring the delivery of appropriate care for a caseload of patients.
Demonstrate advanced knowledge, skills and innovative practice in
relevant specialist area. Ensure that there is a system of nursing
documentation, which reflects this advanced practice. Promote clinical excellence reflecting the Sandwell & West Birmingham Hospitals NHS Trust nursing strategy and business plans.
The post holder will aim to prevent hospital admissions and reduce length of stay, when possible, through providing treatment and support to those with long term respiratory conditions in line with national and local guidelines
