About the role

Your new role:

As the Nurse Practitioner within CAREPACT, you'll use your advanced clinical expertise to provide comprehensive assessment, diagnosis, treatment and care coordination for older patients living in residential aged care. Working autonomously and collaboratively with multidisciplinary teams, you'll help deliver innovative, evidence-based care that improves patient outcomes while supporting integrated healthcare across Metro South Health. In this role you will:

Deliver advanced clinical assessment, diagnosis, treatment and prescribing within your endorsed scope of practice.

Lead person-centred care that reduces avoidable emergency department presentations and hospital admissions.

Collaborate with residential aged care providers, GPs, emergency departments and specialist teams to coordinate seamless patient care.

Drive clinical excellence through leadership, quality improvement, education and evidence-based practice.

About you:

You're an experienced and endorsed Nurse Practitioner who is passionate about improving healthcare for older people. You thrive in autonomous clinical practice while building strong partnerships across multidisciplinary teams and are committed to delivering compassionate, high-quality care. You'll bring:

Advanced clinical expertise in emergency, gerontological or aged care nursing.

Excellent clinical judgement with strong assessment, diagnostic and decision-making skills.

Outstanding communication and collaboration skills with the ability to build effective relationships across healthcare settings.

A commitment to leadership, continuous improvement, research and delivering exceptional patient-centred care.

About us:

The Princess Alexandra Hospital (PAH) is a tertiary health care centre providing care in most major adult specialties. We provide world class health care to Brisbane City's southern suburbs across emergency, medical, mental health and surgical services, including liver and kidney transplant.

We are nationally recognised for our expertise in trauma management and organ transplants and are one of Australia's leading academic and research health centres. The PAH as a major medical research precinct hosts Australia's most advanced medical research facility, the Translational Research Institute (TRI) as well as a new Clinical Research Facility for the discoveries made at TRI to be trialed in a safe clinical environment.

We're expanding our hospital to make sure everyone gets the care they need. We're adding more beds, modernising our buildings, and improving our spaces to treat kidney and cancer patients with all new services and facilities planned to open by 2027.
