Post specific related information

The purpose of the post is to effectively support the implementation of the Age Friendly Health System Proof of Concept in the HSE South West.

The post holder will provide project management and administrative expertise and support successful implementation of the 4M framework in identified clinical settings and across transitions of care.

The post holder will lead and participate in work streams as assigned.
