Why does the job exist?

The Rural Health Transformation (RHT) Healthy Horizons Manager will:

Support implementation and operational oversight of the Healthy Horizons initiative within the New Mexico Rural Health Transformation (RHT) Program;

Support implementation of the RHT governance structure by coordinating with providers, contractors, technology partners, and state agencies to support initiative implementation, stakeholder engagement, and alignment with CMS requirements and State priorities.

How does it get done?

The RHT Healthy Horizons Manager is responsible for:

Coordinating activities related to specialty access expansion, telehealth, remote patient monitoring (RPM), care coordination, and technology-enabled rural care delivery models across rural, frontier, and Tribal communities;

Leading implementation and operational coordination for the Healthy Horizons initiative, including specialty care expansion, telehealth, remote patient monitoring (RPM), provider-to-provider consult models, referral pathways, and community-centered remote care;

Coordinating with providers, hospitals, clinics, technology vendors, contractors, state agencies, and external stakeholders to support approved workplans, operational workflows, and care delivery models;

Coordinating provider engagement activities, learning collaboratives, meetings, and implementation discussions for Healthy Horizons;

Monitoring initiative progress, milestones, operational risks, and performance activities related to access expansion and technology-enabled care delivery;

Developing implementation materials, operational guidance, executive briefings, stakeholder-facing communications, and tracking tools;

Collaborating with Program Management Office (PMO) leadership, finance, procurement, communications, data and evaluation, and operations staff to support aligned implementation across the RHT Program portfolio;

Supporting sustainability planning, reporting, and stakeholder engagement activities for the Healthy Horizons initiative.

Work is performed for the Health Care Authority (HCA), the RHT Program Management Office, providers, hospitals, clinics, technology partners, contractors, state agencies, CMS, and program stakeholders to support implementation and coordination of the Healthy Horizons initiative within the Rural Health Transformation Program.

Minimum Qualification

Bachelor's Degree in a relevant field of study from an accredited college or university and four (4) years of professional level experience with a tactical impact directly related to the purpose of the position defined by the agency at the time of recruitment. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling eight (8) years may substitute for the required education and experience.

Employment Requirements

Must possess and maintain a valid Driver's License.

Must obtain a Defensive Driving Certificate.

Working Conditions

Work is primarily performed in an office setting with extensive use of a computer, phone, virtual meeting platforms, and Visual/Video Display Terminal (VDT) equipment. Position may require prolonged sitting, coordination of virtual and in-person meetings, review of operational and program documentation, and occasional in-state travel for provider engagement, stakeholder meetings, trainings, implementation activities, or site visits.
