What you'll be doing

Kids GPS Care Coordination is a network service designed for children with chronic and complex conditions who frequently utilise hospital services.

Kids GPS aims to improve efficiency and patient outcomes by supporting families to navigate services and providers within the hospital and the child’s local community, integrating tertiary, secondary and primary health. This enables patients and

families to access the right care at the right place at the right time by the right clinician.

The Appointment Coordinator will work collaboratively with the Kids GPS Care Coordinators, and other multidisciplinary clinicians to:

Coordinate care between clinicians and services to minimise travel and the length of time spent away from home, off school and away from employment.

Ensure all relevant information is well communicated between SCHN clinicians, local clinicians and the family.

Provide advice and support to parents/carers regarding travel and accommodation options

Demonstrated ability to manage a workload, prioritise tasks and maintain accuracy and attention to detail.

Demonstrated capacity to work independently, and collaboratively, and exercise initiative and judgement in dealing with confidential issues.

Excellent verbal and written communication skills, with a focus on customer service

Experience dealing with patients and families in a hospital setting

Competency with computer software, including Microsoft suite (Outlook, Word, Excel)

Medical Administrative experience desirable

Permanent NSW Health roles require Australian citizenship or permanent residency. Temporary visa holders may be considered where no suitable Australian citizen or permanent resident is identified.

NSW Health vaccination and screening requirements must be met prior to commencement.

This position requires a Working with Children Check (WWCC) issued by the Office of the Children’s Guardian.

Where required, overseas qualifications must be formally assessed by the Department of Education or relevant authority for recognition in Australia.
