About the role
MEDRITE is seeking a Psychiatric Nurse Practitioner (PNP) to provide high-quality, evidence-based behavioral health care within a dynamic, multi-state urgent and concierge care platform. The PNP will be responsible for conducting psychiatric evaluations, prescribing and managing psychotropic medications, and providing ongoing care management in coordination with medical and behavioral health teams. Services will be delivered via both telehealth from our offices and on-site visits, depending on location and patient need.
Key Responsibilities
Conduct psychiatric assessments and evaluations for patients with behavioral health needs.
Develop and manage individualized treatment plans, including medication management.
Provide care for patients with substance use disorders, including opioid dependence.
Collaborate with primary care providers, behavioral health clinicians, and support staff to ensure continuity of care.
Recommend therapeutic regimens and monitor for side effects and treatment response.
Participate in case reviews, quality assurance activities, and clinical training sessions.
Maintain thorough documentation in MEDRITE’s EMR system in accordance with state and regulatory guidelines.
Support transitional care for patients discharged from psychiatric hospitalization.
Stay current with best practices, emerging treatments, and telehealth innovations in psychiatric care.
Qualifications
Master's degree or higher in Nursing from an accredited Nurse Practitioner program.
Board Certified Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC).
Active, unrestricted Nurse Practitioner licenses in New York, New Jersey and other states a plus.
Data 2000 waiver (X waiver) or eligibility strongly preferred.
Must have Social Care Network Community Experience - Senior Population
Minimum of 2 years of experience in outpatient psychiatry or behavioral health setting.
Prior telehealth experience is preferred.
Proficiency in electronic medical records and virtual care platforms.
Strong interpersonal, organizational, and communication skills.
Comfortable working independently in a fast-paced, evolving care delivery model
Must be able to sit or stand for periods of time
Must eb able to lift up to 50lbs.
