Position Summary
We are seeking a detail-oriented and patient-focused Pharmacy Technician to support our Clinical Pharmacy team in delivering high-quality care and medication management services. This role plays a vital part in coordinating pharmacy-related activities, facilitating communication between providers, patients, pharmacies, and patients, and supporting quality initiatives for our Medicare population. The ideal candidate will possess strong organizational skills, healthcare experience, and the ability to manage multiple priorities in a fast-paced environment.
Key Responsibilities
Provide administrative and operational support to Clinical Pharmacists.
Participate in Pharmacy Quality initiatives by facilitating Nurse Practitioner (NP) appointments when necessary.
Track and monitor pharmacy referrals, interventions, and outcomes.
Track Post-Discharge Medication Reconciliations for Medicare patients.
Perform data entry and assist with chart abstraction and clinical record preparation for Annual Wellness Visits and Post-Discharge Visits.
Obtain clinical documentation and relevant patient information from provider offices to support pharmacy services.
Process Prior Authorizations through electronic and telephonic platforms, including CoverMyMeds, Express Scripts, Medicaid, and other payer systems.
Communicate prior authorization outcomes to pharmacies, providers, and patients in a timely manner.
Conduct follow-up outreach to patients requiring post-discharge pharmacist consultations when initial contact attempts are unsuccessful.
Assist with pharmacy reporting, data collection, and performance tracking.
Support pharmacy analytics and quality improvement projects.
Participate in cross-functional special projects, including Round Table initiatives and other departmental assignments.
Interface with vendors and external partners to obtain necessary information and ensure timely resolution of issues.
Projects and other duties as assigned.
Requirements
High school diploma or equivalent required; Pharmacy Technician certification preferred.
Previous experience in a pharmacy, healthcare, care coordination, or clinical support environment preferred.
Knowledge of medication prior authorization processes and payer requirements.
Familiarity with Medicare populations and post-discharge care processes is a plus.
Strong data entry, documentation, and organizational skills.
Ability to communicate professionally with patients, providers, pharmacies, insurance companies, and vendors.
Proficiency with Microsoft Office applications and electronic health record (EHR) systems.
Excellent attention to detail and ability to manage multiple tasks simultaneously.
Strong problem-solving skills and ability to work independently and collaboratively.

Pay: $54,000.00 - $62,000.00 per year
