JOB TITLE: Home Leader – Supported Living
DEPARTMENT: Community Living
SUPERVISOR'S TITLE: Program Manager(s) – Supported Living Assistant Director - Supported Living
POSITIONS SUPERVISED: Direct Support Professionals (day-to-day supervision)
JOB STATUS: Fulltime, Non-Exempt
SCHEDULE: To be determined; availability for first, second and third shifts required
Summary
Provide day-to-day supervision and oversight of Supported Living home staff while ensuring high-quality, person-centered services for individuals served. This role combines direct support with some supervisory responsibilities to promote independence, health, safety, and meaningful community participation.
Key Responsibilities
Staff Supervision
Provide day-to-day supervision, guidance, and support to Direct Support Professionals (DSPs)
Assist with staff training
Provide coaching, modeling, and performance feedback to staff
Ensure proper staffing coverage and assist with filling open shifts
Assist in staff scheduling
Communicate effectively with Program Managers and leadership regarding staff performance and concerns
Service Delivery & Support of Persons Served
Support individuals in daily living skills, personal care, and household routines
Facilitate community participation, recreation, and social engagement
Provide or coordinate transportation to appointments, employment, and activities
Assist with financial management tasks (e.g., budgeting, tracking expenses)
Promote independence, self-advocacy, and informed decision-making
Build and maintain relationships with families, providers, and community partners
Schedule and attend medical and other essential appointments as needed
Ensure medications are ordered, stored, and administered according to regulations
Provide crisis intervention and emergency support as needed
Ensure the overall safety and well-being of individuals served
Individual Support Plan (ISP) Oversight
Monitor progress and support implementation of ISP goals and objectives
Collaborate with interdisciplinary teams to develop and adjust service strategies
Ensure completion of goal tracking, behavior data, and medication records
Support staff in accurate and timely documentation
Documentation & Administrative Duties
Ensure completion of required documentation, including:
Daily logs and service notes
Medication records and audits
Safety drills and home inspections
Billing and tracking forms
Review documentation regularly to ensure accuracy and compliance
Maintain up-to-date client and staff records per state and agency guidelines
Qualifications & Skills
Strong leadership, communication, and interpersonal skills
Ability to supervise, coach, and support team members effectively
Demonstrated empathy, professionalism, and commitment to person-centered care
Strong organizational and problem-solving skills
Ability to manage multiple responsibilities in a fast-paced environment
Understanding of advocacy and support strategies for individuals with disabilities
Physical & Work Requirements
Ability to work a flexible schedule, including evenings, weekends, and holidays
Ability to work every other weekend
Ability to provide personal care (e.g., toileting, bathing, feeding) with dignity and respect
Ability to assist with transfers and mobility support
Frequent standing, walking, bending, and lifting
On-call availability as required (24/7 support oversight)
Ability to respond effectively in emergency situations
Valid driver’s license, reliable transportation, and proof of insurance
Certifications & Training Requirements
CPR, First Aid, and Bloodborne Pathogens certification
Certified Restraint Training
Core A & B certification
Completion of required agency and regulatory trainings (e.g., Human Rights, Safety, Medication Administration)

$350 Sign-on Bonus eligible position. Must be scheduled a minimum of 20 hours per week for the sign on bonus.
Shift Differentials: Additional $0.50 for 2nd and 3rd shifts on week days; Additional $1.00 for weekends

Requirements
Education/Experience Requirements:
High school diploma or equivalent preferred, but not required. Experience in providing direct service to people with developmental disabilities or experience in a volunteer or teaching capacity with persons with developmental disabilities is desirable.
