About the Role
We are seeking an experienced Hospice Business Development Liaison who understands the hospice landscape and has a proven track record of building referral relationships and driving admissions.
This is not an entry-level opportunity. The ideal candidate brings existing provider relationships, strong market knowledge, and the ability to independently grow a hospice census.
What You’ll Do
Leverage existing relationships with physicians, hospitals, SNFs, and community partners to drive referrals
Actively grow hospice admissions through strategic business development efforts
Position Choice as the preferred hospice provider in the market
Educate referral sources on hospice eligibility, services, and benefits
Collaborate with clinical teams to ensure seamless patient transitions
Execute targeted outreach and community engagement strategies
Consistently meet or exceed monthly admission goals
What We’re Looking For
Required: Proven experience in hospice or home health business development/liaison role
Demonstrated success in meeting or exceeding admission/census goals
Established relationships within the local healthcare community (strongly preferred)
Deep understanding of hospice services, regulations, and referral processes
Highly self-directed, with the ability to manage a territory independently
Valid driver’s license and auto insurance
HomeCare HomeBase experience (preferred)
